FR106 In-Occupancy DAS Furniture Request
This request form is only to be used for In-Occupancy, exchange / return / or new DAS items.
In Occupancy requests should be recorded using this form only. The DAS provider will check your request against your entitlement before making a mutually suitable appointment for delivery/collection. On collections all items are to be presented clean and serviceable and must remain within the SFA. It will be checked by the DAS team for damages. You are then requested to sign the form to acknowledge any damage. Any billing will then be taken as per JSP 384 Chapter 13.
